Humber Meadows Long-term Care Home will be a 320 bed home with approximately 435 employees that is part of the rapid build initiative of the Government of Ontario. The initiative aims to increase the LTC bed capacity in Ontario as well as to support the modernization of care in LTC. The model of care in the home will be inspire by the Butterfly Model and aim to become a high reliability home as well as a driver of innovation and quality improvement to support its vision of building the future of senior care. Home is scheduled to open early 2023.

 

Position Summary:

The Medial Director reports to the Executive Director, and is responsible for clinical leadership and to advise on medical services and programs at HMLTC. The Medical Director provides oversight of Attending Physicians, and ensures all clinical programs and services meet both MLTC and Ontario Medical Association policies and regulations. The Medical Director ensures all regulatory and best practice guidelines are followed and is responsible for the overall care and clinical practice carried out at HMLTC. The Medical Director also provides information that helps others (including team members, residents & families) understand the care medical interventions and recommendations

 

Qualifications:

  • Medical Doctor with 3-5 years experience;
  • Attending Physician in Long-Term Care for at least 1 year
  • Experience working with residents/loved ones in long term care
  • Experience with MediSystem, eMar, eConnect
  • Thorough knowledge of MDS and Resident Care Plan review/updating
  • Highly developed interpersonal skills and demonstrated ability to work effectively as part of a team
  • Proven adherence to strict confidentiality
  • Demonstrated excellence in effectively organizing and prioritizing work assignments in a fast-pace office environment and meeting short timelines

 

Major Responsibilities:

  • Provides advice and leadership on all medical services and programs;
  • Ensures regular attendance and presence at HMLTC while maintaining support of Attending Physicians and model of care;
  • Willingness to act as, or provide coverage for, Attending Physicians at HMLTC;
  • Supports the development of care standards in conjunction with HMLTC and the interdisciplinary team;
  • Provides medical expertise and support for complex resident medical needs, including but not limited to, acute and chronic diagnosis, palliation and adverse medical episodes;
  • Oversees the monitoring, measurement and quality improvement of medical care at HMLTC, while supports in the development of medical strategies for exceptional resident centred care;
  • Commit to continuing medical education and maintaining current knowledge and practice in geriatrics and gerontology, and supporting associating education opportunities with Attending Physicians and nursing teams;
  • Provide, or arrange for medical services to be provided, for all residents who do not have an Attending Physician identified at HMLTC;
  • Provide medical liaison with local hospital(s) and other appropriate health care organizations or facilities, for all prospective and current residents;
  • Works effectively and efficiently with members of the interdisciplinary team including the Attending Physicians to ensure the highest level of care is provided to residents;
  • Regularly communicates with attending physicians on relevant policies, procedures and responsibilities;
  • Ensures appropriate on-call coverage has been arranged for all residents, after hours, in accordance with LTC on-call program as established between MLTC and Ontario Medical Association;
  • Ensures input and oversight in the selection of Attending Physicians; is a key contact for all concerns related to attending physicians;

Position Summary:

This position reports to the Medical Director, in conjunction with the Executive Director and Director of Care, The Attending Physician is responsible for providing exemplary resident-centered medical care and would support a roster of approximately 60 residents. The Attending Physician will work closely with the registered staff on the Resident Home Areas, as well as the clinical leadership team. The Attending Physicians will work together to ensure Physician On-Call Coverage and a resident-centered approach to changing needs.

Qualifications & Experience:

  • Medical practitioner licensed to practice medicine in the Province of Ontario, and a member in good standing of the Colleges of Physicians and Surgeons of Ontario (CPSO)

  • Experience working in LTC and Knowledge of legislations regulations governing LTC (e.g. Fixing Long-Term Act 2021 and Ontario Regulation 246/22) an asset

  • Experience with PointClickCare, eMar, eConnect and Secure Conversations

  • Thorough knowledge of MDS and Resident Care Plans

  • Ability to build trust and interact in a professional manner with residents and their families as well as team members

  • Highly developed interpersonal skills and demonstrated ability to work effectively as part of a team

Duties & Expectations:

  • Attends the LTC home on a weekly basis, to provide clinical care and conduct assessments with assigned residents;

  • Addresses resident acute and chronic care needs and provides appropriate and timely assessments and documentation in the electronic health record;

  • Identifies circumstances in which palliative and/or end of life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team;

  • Works effectively and efficiently with members of the interdisciplinary team including the Medical Director to ensure the highest level of care is provided to residents;

  • Conducts regular medication drug review and documents/updates resident’s health record;

  • Commits to continuing education related to long term care, care of the elderly, palliative care and dementia care;

  • Participates in the after hour on-call rotation.
